Please do not return these items to Faith Christian School. Everything goes to CSTAS
The deadline to apply for tuition assistance is March 15, 2010.
Remember to include the following with your application:
• The completed CSTAS form • Your check or money order for $17.00 payable to CSTAS • Your W-2’s for all employers • Your 2009 Federal income tax returns • Documentation of non-taxable income
In order to be considered for financial assistance, parents must complete the 2010-2011 enrollment or re-enrollment forms which are available in the school office (a copy was also mailed with the report card in December) or off the school website (www.faithchristian.info) with this cover sheet – this lets us know you have applied for financial assistance. Return these 2010-2011 enrollment or reenrollment forms to the school office during January or February. The School Board will consider tuition assistance applications at the April, 2010 meeting. The school office will contact those who applied after that meeting regarding the status of their application for assistance.
